I had been working with a simple comment form like above this week, and I challenged myself today to add title Mr./Ms., but it turned out more difficult than I imagined. I encountered with some bugs, and it took almost a half day to fix it ><!
Finally, I solved this issue by asking on Stackoverflow.com, and my subject got a big access hit and received many helps! This site is very helpful and useful, and there are reliable specialists/experts there!
